Arizona Pet Pantry Guidelines

*Our food assistance program is for those in a current financial crisis.
*Arizona Pet Pantry staff will come for a home visit as part of the approval process.
*One application per household.
*Identification will be required.
*Applicant must be at least 62 years old and we only serve southern Arizona.
*Once approved, the assistance is good for up to 30 days, depending on the need. After 30 days, the situation will be reevaluated.
*Proof of current financial crisis may be required.
*Proof of Spaying and Neuter and current vaccinations will be required.

By submitting this request, you are stating that all information on this application is true, and understand that any false information may disqualify you from receiving assistance through Arizona Pet Pantry.
By submitting this request, you are releasing Arizona Pet Pantry and its sources from any liability arising from the condition of the food and further agree to indemnify and hold harmless Arizona Pet Pantry and its sources against any and all damages, losses, claims, causes of actions and suits of law or any obligation whatsoever arising out of or attributed to any action in connection with the food.
By submitting this request, you are agreeing to all guidelines and stipulations required by Arizona Pet Pantry to receive pet food assistance.

*After you submit your application a representative from Arizona Pet Pantry will contact you within 72 hours (3 days).  Please DO NOT SUBMIT MORE THAN 1 APPLICATION per household and understand that we will get back to you as soon as possible, within the 72 hour time frame we have established.  Thank you.
